Job summary

Amedisys Home Health, Homewood, AL, is hiring a Full Time Licensed Practical Nurse for a Monday–Friday, 8am–5pm schedule, delivering in‑home patient care and coordinating with the care team.

You will verify plans of care with the RN, monitor vitals, manage medications, teach self‑care, perform procedures, and document accurately. Orientation pay is $26–$35/hour, then switch to a per‑point pay method. Benefits include health plans, PTO, 401(k) with match, and wellness support.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver\'s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Attractive pay

$26.00 to $35.00 per hour for Orientation and will then convert to a per point pay method.
