Job summary

AMEDISYS HOME HEALTH is seeking a licensed practical nurse for PRN weekday visits in Winchester, TN. The role involves verifying plans of care, coordinating with physicians, teaching patients self-care, and performing essential nursing tasks in patients’ homes.

You will assess vital signs, reconcile medications, and assist with treatments while maintaining accurate documentation and communicating patient changes to the care team.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them promptly when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
