Job summary

Amedisys is hiring a licensed practical nurse to join our home health team in Nashville. This full-time role offers weekday hours from 8AM to 5PM, with on-site patient visits and collaborative care with RNs and physicians.

You will verify plans of care, monitor vitals, perform medication reconciliation, and educate patients and families. The position requires 1 year of LPN experience, current state license, CPR, and a valid driver's license.

Qualifications

• At least one year of clinical LPN experience.
• Current LPN license for the state(s) you will work.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
