Qualifications

• Current LPN license for the state(s) you will work in.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to address patient needs.
• Teach patients and caregivers self-care techniques; check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
