Job summary

Amedisys, Inc. is recruiting a licensed practical nurse to join our home health team in Florida. The role focuses on delivering patient-centered care in patients' homes, with emphasis on safety, education, and coordination with clinicians.

The LPN will verify plans of care, monitor vitals, reconcile medications, and document visits while ensuring high-quality, compassionate service to patients in their own homes.

Qualifications

• One year of clinical LPN experience.
• Current LPN license valid for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform reconciliation.
• Perform procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment using aseptic technique.
• Complete documentation timely and accurately.
