Qualifications

• Bachelor’s degree in physical therapy and valid PT license.
• Licensed Physical Therapist (PT) (IDFPR) in Illinois or applicable state.
• Basic Life Support (BLS) certification by the American Heart Association.
• Continuing education commitments for acute stroke care.

Responsibilities

• Performs patient transfers with proper body mechanics and safety.
• Delivers compassionate patient care with dignity and respect.
• Develops and adjusts plan of care with goals and discharge planning.
• Educates patients, families, and caregivers using effective methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Documents examinations, treatment plans, and progress in EMR.
• Screens patients to determine therapy needs and duration.
• Coordinates with support personnel and supervising clinicians.
