Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages in Illinois. The role involves developing care plans, documenting outcomes, and coordinating with the care team.

Qualifications

• Licensed Physical Therapist (PT) in Illinois.
• Basic Life Support (BLS) within 30 days - American Heart Association.
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

Responsibilities

• Demonstrates ability to transfer patients with assistance and uses proper body mechanics.
• Treats patients with dignity, empathy, and engagement.
• Designs and adjusts a plan of care based on goals, outcomes, and discharge plan.
• Educates patients, families, caregivers, and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations and documents results in the EMR.
