Job summary

Affinity Health Management seeks a NJ Certified Hospice Aide to work under RN supervision, performing patient care per the plan of care and documenting observations. The role emphasizes comfort, safety, and quality of care in a hospice setting.

Responsibilities include personal care, communication with patients and families, safety practices, and accurate reporting to the RN Case Manager. A valid HHA license and driving eligibility are required.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• 1 year previous home health/hospice experience preferred.
• Basic computer skills to document in electronic medical records (EMR).
• At least 18 years of age.
• Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Responsibilities

• Perform personal care needs after a patient assignment and attend to requests promptly.
• Provide patient and family with respectful communication and confidentiality.
• Meet safety needs of patient and use equipment safely.
• Give personal care including baths, back rubs, hygiene, and linen changes.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned.
• Observe and report patient condition changes to RN Case Manager.
• Assist in ambulation and positioning as directed.
• Support self-administered medications as allowed by state regulations.
• Assist with light housekeeping tasks and bedpans/urinals as needed.
• Maintain clean and orderly living area and comply with procedures.
