Job summary

Affinity Health Management is seeking a hospice aide to provide compassionate, supervised personal care under an RN. You will report observations, document care, and support safety and comfort for patients, in home or facility settings.

Required CNA course, NAC, CPR, and a high school diploma or GED; hospice or long-term care experience preferred; valid driver with insured vehicle is a plus.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) certification.
• Current Healthcare Provider CPR certification; hands-on demonstration preferred.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Prior hospice or long-term care experience preferred.

Responsibilities

• Perform personal care tasks as assigned and respond promptly to patient requests.
• Communicate with patients and families with empathy and confidentiality.
• Ensure safety and proper use of equipment; assist with mobility and transfers.
• Provide baths, oral hygiene, grooming, and changing bed linens as needed.
• Assist with feeding and monitor for issues; no home-made foods.
• Document observations and care in EMR and report changes to RN Case Manager.
• Assist with range of motion, positioning, and basic exercises per instructions.
• Support with medication reminders as allowed by state regulations.
• Maintain a clean living area and assist with light housekeeping tasks.
