Job summary

Affinity Health Management seeks a compassionate hospice aide to assist with personal care under the supervision of an RN. You will perform tasks outlined in the patient’s plan of care to promote comfort and report observations to the Case Manager.

Responsibilities include bathing, grooming, mobility, meals assistance, safety, documentation, and communication with patients and families. Prior CNA or hospice experience preferred; must meet state requirements and maintain professionalism.

Qualifications

• Completion of state-approved CNA course or equivalent training.
• NAC certification.
• CPR certification (in-person skills) as required.
• Food handlers permit if applicable.
• Higher school diploma or GED.
• Ability to follow written instructions and document care.

Responsibilities

• Provide personal care (baths, grooming, hygiene) and assist with daily activities.
• Assist with feeding and mobility; ensure safe transfers and positioning.
• Observe and report changes in patient condition to RN Case Manager.
• Maintain cleanliness of patient living area and basic light housekeeping.
• Document care provided and communicate with family with sensitivity and confidentiality.
• Support with medication reminders as allowed by state regulations.
• Participate in team quality improvement activities.

Education

High school diploma or GED

State approved CNA course

NAC (Nursing Assistant Certified)

CPR certification

Basic computer skills (EMR)

Licensed driver with insured vehicle

Ability to read and follow instructions
