Job summary

CENTERVILLE in seeks a Speech-Language Pathologist to evaluate and treat patients, communicate with families and physicians, and maintain documentation in medical records. The role includes supervising CFY/CFY-RFP fellows and providing staff training as part of the facility’s rehabilitation program.

Responsibilities include timely patient evaluations, creating treatment plans, clinical documentation, and participating in care conferences and training sessions for staff.

Qualifications

• Education and/or Experience: a master's or doctoral degree in SLP and licensure as an SLP.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral, documenting delays.
• Develop effective treatment plans and obtain physician approval.
• Treat patients per the physician plan and supervise CFY/CFY-RFP fellows.
• Communicate patient progress with supervisors and health team members.
• Participate in care conferences and rehabilitation meetings.
• Record evaluations and daily/weekly progress notes according to policy.
