Position Description

Performs advanced, specialized work to assist in the organizationand implementation of various projects and programs. Work isperformed under minimal supervision within established policieswith latitude for independent judgment. Works on special projectsas assigned. May substitute for supervisor during his/herabsence.

Major/Essential Functions

• Establish close working relationships with patients and membersof an interprofessional team to identify non-medical drivers ofhealth and reduce health disparities.
• Promote goal setting and strategic planning for the bettermentof overall wellness.
• Accept referrals and provides case management for Children andPregnant Women (CPW), and chronic disease management for IPCP.
• Asses healthcare needs to provide care coordination, tailoredhealth educ and identifies community resources.
• Serve as patient advocate and assists in identification andimprovement of service delivery.
• Document case management activities in the electronic healthrecord.
• Assist in the organization and implementation of variousprojects and programs.
• Perform work under minimal supervision within establishedpolicies with latitude for independent judgment.

Occasional Duties

• Develop and present training programs for community members,patients, and families.
• Participation in research related activities.
• Substitute for supervisor during their absence.

Required Qualifications

High school plus six years experience in related field. Additionaleducation may substitute for experience on a year for yearbasis.

Preferred Qualifications

• Certified Community Health Worker (CHW) or currently enrolledand attending a CHW program that will be completed within sixmonths of employment.
• Experience with application of procedures within the CHW scopeof practice.
• Experience in dealing with behavioral health issues and/ornavigating the behavioral health system.
• Experience in developing relationships with individuals withserious mental illness and/or substance use disorder, and ademonstrated ability to connect with and support thispopulation.
• Experience with EPIC software.
• Experience in data collection.
• Experience in conducting community referrals for clients.
• Community outreach experience.
• Ability to communicate effectively in Spanish and English.
