Job summary

ChenMed is seeking a Primary Care Physician (PCP) to deliver comprehensive care to an aging patient panel of up to 450. You will provide high-quality, cost-conscious care, build trust with patients, and lead a care team to improve health outcomes.

This physician-led, value-based organization emphasizes relationship-centered care, coaching patients toward goals, and collaborating with hospitalists, APPs, and managers to align with center and market goals.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics.
• Must obtain/possess a State Medical License for the applicable state.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• Board Eligibility is required; maintain board certification in their specialty.
• Current DEA number for schedule II-V controlled substances.
• BLS certification from AMA or Red Cross within first 90 days.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Engage with hospitalists and case managers for inpatient and transition care.
