Job summary

ChenMed is seeking a Primary Care Physician (PCP) to join our physician-led, mission-driven team. The PCP will manage a panel of up to 450 patients, deliver high-quality care, and partner with care teams to achieve value-based outcomes.

You will lead a care team, emphasize evidence-based medicine, and participate in risk management and quality initiatives. A valid MD/DO in relevant fields and licensure are required.

Qualifications

• MD or DO in Internal Medicine, Family Medicine or Geriatrics and state licensure.
• Board certification preferred; board eligibility required.
• Ability to obtain/maintain medical license in designated states.

Responsibilities

• Provide comprehensive care for adults, including acute and chronic conditions.
• Lead a care team and coordinate services for optimal patient outcomes.
