Qualifications

• Bachelor's degree in Physical Therapy and licensure required.
• Licensed Physical Therapist in the state; maintain licensure.
• Computer skills with Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral.
• Develop and obtain approval for treatment plans from referring physicians.
• Supervise PT assistants and provide direct patient care supervision.
• Record evaluations and daily/weekly progress notes per policy.
• Discharge planning and caregiver training prior to discharge.
• Participate in interdisciplinary meetings and in-service trainings.
• Order and facilitate durable medical equipment as needed.
• Communicate progress with supervisor and health team.
