Job summary

Qualifications

• Bachelor's degree in Physical Therapy; advanced degree preferred.
• Licensed Physical Therapist in the state; maintain license.
• Proficient with Casamba and Point Click Care.
• Strong ability to read and follow policies and procedures.
• Ability to interpret care plans and documentation.

Responsibilities

• Evaluate patients within 24 hours of referral and document delays.
• Develop and obtain approval for treatment plans from physicians.
• Supervise PT assistants and student trainees.
• Document daily and weekly treatment notes per policy.
• Communicate patient progress with supervisor and health team.
• Participate in care conferences and in-service trainings.
• Record discharge planning and manage therapy documentation.
• Assist in ordering durable medical equipment as needed.
