STONERIVER is seeking a licensed Physical Therapist to evaluate patients within 24 hours of referral and develop approved treatment plans. You will supervise PT assistants and students, coordinate with the health team, and participate in discharge planning and conferences.

You will document evaluations, progress notes, and recertification, and assist with training and equipment ordering to promote patient independence.

Qualifikationen

• Licensed as a Physical Therapist in the state.
• Masters or Doctorate in Physical Therapy preferred.

• Evaluate patients within 24 hours of referral.
• Develop and obtain approval for treatment plans.
• Supervise PT assistants and students.
• Record evaluations and treatment notes per policy.
• Discharge planning involvement.
• Communicate progress with supervisor and health team.
• Instruct families and staff in maintenance and discharge prep.
• Facilitate ordering of durable medical equipment.
• Participate in care conferences and in-services.
