Job summary

Riverview is seeking a licensed Physical Therapist to evaluate and treat patients, supervise PT staff, and participate in discharge planning within a skilled nursing or rehabilitation setting in Ohio. The ideal candidate holds a Bachelor's or higher degree in Physical Therapy, is licensed in the state, and proficient with Casamba and Point Click Care.

Qualifications

• Licensed as a Physical Therapist in the state.
• Proficient in Casamba and Point Click Care.
• Ability to read and comprehend policy and procedure manuals.
• Effective communication with managers, coworkers and families.
• Ability to supervise PT assistants and students.

Responsibilities

• Evaluate patients within 24 hours or 1 business day after referral.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per the physician plan and document progress.
• Supervise PT assistants, aides and students in direct care.
• Record evaluations and treatment notes per PT Board Act and payer rules.
• Participate in discharge planning and family education for home care.

Education

Bachelor's degree in Physical Therapy

Masters or Doctorate in Physical Therapy (preferred)
