Job summary

21st Century Home Health Services in California seeks a Physical Therapist Case Manager, Level 2, to evaluate and treat patients in their homes and supervise two PTAs. You will coordinate with physicians to ensure high-quality care and restore patient function.

Qualifications

• Current unrestricted CA Physical Therapist License is required.
• 1+ year experience supervising PTAs in a home health environment.
• Current BLS/CPR certification required.
• Valid CA driver's license required.
• Knowledge of local community resources preferred.

Responsibilities

• Perform home-based PT evaluations and develop individualized care plans.
• Supervise PTAs and ensure performance standards are met.
• Coordinate with physicians to ensure high-quality patient care.
• Evaluate patient progress and adjust plans to maximize function.
• Create and implement detailed treatment plans for each patient.
• Communicate with care team to maintain continuity of care.
