Job summary

21st Century Home Health Services is seeking a Physical Therapist Case Manager, Level 2, to evaluate and treat patients in their homes throughout American Canyon, CA. You will supervise 2 PTAs, develop care plans, and coordinate with physicians to restore function and promote independence.

Qualifications

CA license as Physical Therapist is required.
Minimum 1 year supervising PTAs in a home health setting preferred.
Current BLS/CPR certification is required.
Valid CA driver's license and reliable transportation.
Ability to coordinate with an interdisciplinary team and physicians.

Responsibilities

Perform home evaluations and develop care plans; supervise PTAs.
Help patients regain mobility and independence in home setting.
Create detailed treatment plans and progress reports.
Coordinate with physicians to ensure high-quality care.
Document patient progress and outcomes in charting system.
