Enhabit Home Health & Hospice is seeking a registered nurse in a leadership role to coordinate home health services and ensure high-quality patient care. The position involves reviewing care documentation, collaborating with physicians and caregivers, and coordinating scheduling to maintain seamless care delivery.

The role requires an RN license, CPR certification, and prior home health experience, with a preference for bilingual candidates and those with supervisory background.

Qualificações

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements or compact license.
• Two years nursing experience within the last 36 months; one year in home health.
• CPR certification.
• Valid state driver's license and automobile liability insurance.
• Intermediate tech skills (Microsoft Office: Word & Excel).
• Preferred: managerial experience; bilingual English/Spanish.

Responsabilidades

• Coordinate home health services delivered to patients per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation start information.
• Facilitate communication among physicians, referral sources, patients, caregivers, and staff to ensure care coordination.
• Schedule patients to field clinicians and maintain care continuity.
