Enhabit Home Health & Hospice seeks a registered nurse to coordinate comprehensive home health services ensuring care meets standards and procedures. The role includes reviewing patient information for start of care, recertification, resumption of care, and evaluations, and coordinating with physicians, referrals, and caregivers.

The position requires an active nursing license, CPR certification, and at least two years of recent nursing experience, including home health.

Qualifications

• RN degree from an approved nursing program or state licensing authority.
• Must have and maintain a valid nursing license per state requirements.
• CPR certification required.

Responsabilités

• Coordinate overall home health services to patients per standards and procedures.
• Review and approve patient information for start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain care coordination.
