Job summary

Enhabit Home Health & Hospice is seeking an RN leader to coordinate home health services, ensure regulatory compliance, and enable seamless communication among physicians, patients, and staff. A valid nursing license and CPR are required, with at least two years of recent nursing experience and one year in home health.

Preferred candidates have Homecare Homebase (HCHB) experience, strong Microsoft Office skills, and bilingual English/Spanish abilities.

Qualifications

Must graduate from an approved school of professional nursing.
Must maintain a valid nursing license per state requirements.
Experience practicing under the license for this position.
Minimum two years of nursing experience within the last 36 months.
Minimum of one year of home health experience.
Knowledge of federal, state and local home care laws/regulations.
CPR certification required.
Valid state driver license and automobile liability insurance.
Basic technology skills and comfort with mobile devices.
Preferred management experience.
Preferred Homecare Homebase (HCHB) experience.
Bilingual English/Spanish preferred for clear communication.
Dependable transportation in good condition; safe driving.
Oral and written communication proficiency; English fluency.
Intermediate Microsoft Office skills (Word, Excel).
In-office attendance is essential; remote work not typically allowed.

Responsibilities

Coordinate delivery of home health services per standards and procedures.
Review and approve patient information at start of care, recertification, or evaluations.
Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
Schedule patients to field clinicians and maintain communication with involved parties to ensure continuity of care.
