Job summary

Enhabit Home Health & Hospice seeks an experienced Registered Nurse to oversee home health services, ensuring compliant, high-quality patient care. You will coordinate clinicians, physicians, patients, and caregivers while maintaining schedules and regulatory adherence.

The ideal candidate holds a BSN or higher, with CPR certification and 2+ years of nursing in home health. Strong communication in English and Spanish is preferred, and reliable transportation is required.

Qualifications

• RN licensure and CPR certification required.
• Minimum two years of nursing experience in home health or related setting.
• Experience with home health operations and regulatory guidelines preferred.
• Must have dependable transportation and valid state driver license.

Responsibilities

• Coordinate home health services to ensure high-quality patient care.
• Review and approve patient records during start of care, recertification, and care evaluations.
• Facilitate communication between physicians, patients, caregivers, and staff.
• Schedule patients with field clinicians to maintain care continuity.
