Job summary

Enhabit Home Health & Hospice is seeking a qualified nurse leader to coordinate comprehensive home health services and ensure care aligns with standards and procedures. You will review patient data at key milestones and act as a liaison among physicians, referrals, patients and caregivers.

The role requires a valid nursing license, CPR, a driver license with insurance, and experience with home health. Bilingual abilities and proficiency in MS Office are a plus; in-office presence is expected.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license per state requirements or compact license.
• Minimum 2 years nursing experience in past 36 months.
• Minimum 1 year home health experience.
• Knowledge of federal, state and local home care regulations.
• CPR certification and valid driver license with auto liability insurance.

Responsibilities

• Coordinate overall home health services to meet standards and procedures.
• Review/approve patient information at start of care, recertification, resumption of care or evaluation visit.
• Facilitate collaboration among physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination and communication.
