Front St Master is seeking a Licensed or Registered clinical Lead Care Manager to oversee ECM services for Medi-Cal members with complex medical, behavioral health, and social needs. The role emphasizes in-person care management, coordination with the care team, and adherence to CC A H and DHCS requirements.

You will conduct comprehensive assessments, develop care plans, and provide transitional care after discharge, with telehealth as a supplement.

Qualifications

• One year of experience serving adults with serious mental illness, substance use disorders, chronic health conditions, or homelessness.
• Active California clinical credentials listed under Eligible Clinical Credentials.
• Experience documenting Medi-Cal billing.
• Bilingual English/Spanish preferred.

Responsabilités

• Lead care management for an ECM caseload coordinating physical health, behavioral health, and social services.
• Engage eligible members, primarily in person at places of residence or care access.
• Develop, update, and monitor Care Management Plans with member and support persons.
• Provide transitional care after hospital/ED or facility discharge and coordinate with discharging facilities.
• Maintain documentation in the EHR with accurate times and modality.
• Collaborate with interdisciplinary teams to optimize outcomes.
