Job summary

Pacific Health Group is seeking an RNClinical Case Consultant to lead ECM-related care coordination and clinical oversight. The role focuses on high-risk Medi-Cal members, guiding interdisciplinary teams, and ensuring safe, person-centered care across the CalAIM ECM framework.

Hybrid CA position with field visits and collaboration with hospitals, providers, and community partners. The role emphasizes clinical guidance, documentation, and continuous quality improvement within the ECM program.

Qualifications

• Active and unrestricted RN license in California.
• Minimum 2 years direct clinical experience in community health, managed care, or related settings.
• Experience with Medi-Cal populations and complex medical, behavioral health, and social needs.
• Experience supervising LVNs or clinical staff preferred.

Responsibilities

• Provide clinical leadership and consultation to ECM care teams.
• Review complex member cases and provide care planning recommendations.
• Monitor high-risk members and support care transitions.
• Conduct quality assurance reviews of field activities and ensure compliance with ECM standards.
• Lead field visits and supervision of LVNs and CHWs to maintain program quality.
