Job summary

Simple Solutions Psychotherapy is seeking a Case Manager II — ECM/CS (Lead Care Manager) to coordinate care for Medi-Cal members with complex needs. You will lead care planning, assessments, and cross-system collaboration with providers and community partners.

Qualified candidates have an MSW or a related bachelor’s degree with 3+ years of progressive case management, and bilingual English/Spanish is preferred. California driver’s license, LiveScan, and TB clearance are required.

Qualifications

• MSW preferred or Bachelor’s degree with 3+ years of progressive case management experience.
• Bilingual (English/Spanish) preferred.
• Valid California driver’s license, reliable transportation, and auto insurance required. LiveScan and TB clearance required.

Responsibilities

• Lead ECM Lead Care Manager for CalAIM ECM with primary accountability for the member's Care Plan and ICT coordination.
• Coordinate complex transitions of care from hospitals and facilities.
• Lead interdisciplinary care team meetings with primary care, behavioral health, and community partners.
