South Central Family Health Center is seeking a Lead Care Manager for the Enhanced Care Management (ECM) program in the Los Angeles area. The role coordinates organization‑wide ECM services, collaborates with medical, dental, behavioral health, and vision teams, and develops personalized care plans addressing diverse patient needs.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s Degree preferred.
• Medical Assistant/LVN certification preferred.
• Knowledge of community resources in the area.
• Minimum of 2 years’ experience in case management in a community clinic or FQHC setting.
• Comfortable working with diverse populations.

Responsibilities

• Oversees and implements ECM services and pursues Care Plan goals with members.
• Provides services where ECM members live or seek care within guidelines.
• Advocates for members and coordinates engagement, assessment, and education visits.
• Uses motivational interviewing and trauma informed care to develop Care Plans.
• Coordinates with ECM partners to gather data for updates and reporting.
• Links members to education, wellness programs, and community resources.
• Translates for staff when necessary.
• Uses NextGen and other systems to coordinate services and input data for reporting.
• Maintains HIPAA compliance and ECM documentation.
