Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, or related field plus relevant case management experience.
• Experience in permanent supportive housing, homelessness services, behavioral health, or affordable housing preferred.
• Mentor or coach peers; formal supervisory role not required.

Responsibilities

• Direct case management with assigned residents, including engagement, assessment, and goal reviews.
• Coordinate with Property Management and external providers for housing stability and services.
• Document care plans, HMIS entries, and incident reporting with accuracy and timeliness.
• Provide hands-on support for complex or escalated cases and staff coaching as needed.
• Ensure compliance with program requirements and reporting standards.
