Job summary

Mentors Inc Ltd is hiring a Case Manager in Los Angeles to support participants facing housing crises through intensive case management and clinical engagement. You will develop housing plans, coordinate subsidy opportunities, and collaborate with internal teams and external partners to secure permanent housing for families.

The role requires conducting intake, building individualized service plans, and providing advocacy and crisis intervention as needed.

Qualifications

• Bachelor’s Degree in Social Work or related field, or equivalent experience.
• Experience in case management or housing programs preferred.
• Bilingual abilities a plus.

Responsibilities

• Maintain all case files and records in physical files and CHAMP system; complete required documentation and notes.
• Develop and implement housing plans and participant budgets for each program participant.
• Coordinate with Housing Authority, DMV, DPSS, landlords, and community partners to secure housing and subsidies.
• Conduct intake and assessments, gather housing, benefits, and health information to build case plans.
• Provide advocacy, counseling, and crisis intervention for participants experiencing homelessness or housing instability.
