Qualifications

• Associate degree in social services, human services, behavioral health, or related field, or two years of relevant case-management or social-services experience.
• Knowledge of Housing First, harm reduction, trauma-informed care, and participant-centered service delivery.
• Proficient with CHAMP/HMIS and MS Word/Excel; ability to document accurately and timely.

Responsibilities

• Outreach, intake, and engagement with participants.
• Develop and monitor individualized Care Plans within 30 days of enrollment.
• Coordinate housing navigation, subsidies, and move-in processes.
• Maintain regular contact and provide documented ICMS services monthly.
• Ensure compliant documentation in CHAMP/HMIS and required reporting.
