Job summary

Many Mansions seeks a Case Manager to provide on-site intensive case management for residents transitioning out of homelessness in California. The role focuses on assessments, service planning, and coordination with community partners to promote housing stability and well-being.

Ideal candidates have a related degree or multiple years of experience in social services, strong English skills, and the ability to engage empathetically with residents. Driving is required for this position.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related work and 2 years' work experience.
• OR Four years of work experience in mental and behavioral health, social services, social work, case management, and/or counseling.
• Knowledge of Housing First, Harm Reduction, Motivational Interviewing, Strengths-Based Case Management, and Trauma-Informed Care is preferred.
• Must have excellent communications skills, including the ability to speak, write and read English fluently; Spanish-speaking ability is highly preferred.
• Must have strong relationship-building skills and problem-solving abilities to manage complex cases.
• Proficiency in Microsoft Office and HMIS data management.

Responsibilities

• Provide on-site intensive case management to an assigned caseload of residents transitioning out of homelessness.
• Develop and monitor Individual Service Plans and coordinate with community-based organizations.
• Maintain compliance with governmental reporting requirements and keep organized records.
• Collaborate with Property Management to ensure residents' housing stability.
• Coordinate services with Continuum of Care, HMIS, and other agencies.
• Provide life-skills guidance and workshops for residents; maintain daily activity calendars.
• Maintain confidentiality and respond to crises using de-escalation techniques.
