Job summary

PATH is seeking a Lead Case Manager for the Interim Housing team in Los Angeles, CA. The role supports clients in moving from street to housing, maintains a direct client caseload, and collaborates with a multidisciplinary team to meet housing goals.

The position requires 3+ years in healthcare or homeless services, strong assessment and documentation skills, and ability to work in a fast-paced environment. On-site schedule, driving required, benefits offered.

Qualifications

• Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
• Experience developing Individualized Service Plans (ISP) and coordinating housing goals.
• Strong documentation and data interpretation skills.
• Ability to collaborate with multidisciplinary teams and stakeholders.
• Knowledge of trauma-informed care and motivational interviewing is preferred.
• CPR/First Aid training must be completed or obtainable as a condition of hire.

Responsibilities

• Ensure programs comply with contracts and partnership agreements.
• Serve as contact representative for program matters.
• Provide administrative oversight of the team, including potential coverage for the program manager.
• Support completion of critical incident reports per PATH policies or contracts.
• Conduct intake and develop ISP to address housing goals, income, and supports.
• Provide referrals and connect clients to resources to achieve ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT).
• Maintain documentation standards and complete client files with required documents.
• Develop therapeutic relationships to engage clients in housing goals.
• Provide crisis intervention and risk assessment services to enhance safety.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain knowledge of community resources to deliver wrap-around services.
