Job summary

PATH is seeking a Case Manager in Los Angeles, CA to help clients break the cycle of homelessness through individualized services, intake, ISP planning, and coordinated entry. The role focuses on housing goals, income support, and access to necessary health and social services.

You will provide referrals, complete assessments (e.g., VI-SPDAT), maintain careful documentation, and uphold confidentiality while applying motivational interviewing and trauma-informed care to improve housing outcomes.

Responsibilities

• Conduct intake and develop an ISP addressing housing goals and income.
• Provide referrals and support to resources to achieve ISP goals.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate strengths/challenges and conduct a 90-day review with participants.
• Maintain documentation per PATH policies and confidentiality.
• Develop relationships to engage housing goals and provide wrap-around services.
• Provide crisis intervention and risk assessment.
• Use motivational interviewing and trauma-informed care.
• Maintain knowledge of community resources for comprehensive services.
