Job summary

PATH is hiring a Case Manager to join the Interim Housing team in Los Angeles, CA. This full-time role focuses on helping clients break the cycle of homelessness, coordinating services across healthcare, social services, and housing systems.

You will conduct intake, develop individualized service plans, provide referrals, and support clients through interim and permanent housing stages. Driving is required; eligibility and a commitment to social justice are essential.

Responsibilities

• Conduct intake and individualized needs assessment and develop ISP addressing housing goals and income.
• Provide referrals and connections to resources to support ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Conduct 90-day reviews and maintain accurate participant files.
• Maintain confidentiality per HIPAA and guidelines.
• Develop therapeutic relationships to support housing goals.
• Provide crisis intervention and risk assessment.
• Utilize motivational interviewing and trauma-informed care.
• Maintain knowledge of community resources for wrap-around services.
