PATH is hiring a Case Manager in Los Angeles, CA to support individuals experiencing homelessness through intake, individualized service planning, and ongoing housing navigation. Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and connections to resources to meet ISP goals.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate progress and conduct 90-day reviews with participants.
• Maintain accurate participant files and confidentiality per HIPAA.
• Develop therapeutic relationships and connect clients with services.
• Provide crisis intervention and risk assessment to enhance safety.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain knowledge of community resources and coordinate wrap-around services.
