Job summary

The People Concern is hiring a Lead Case Manager to work on the LEAD program in Santa Monica. This role focuses on engaging chronically homeless clients, coordinating connections to housing, health, and support services, and ensuring follow-up after placement.

You will work closely with the LEAD team to prevent jail exposure through proactive case management. You will model wellness and recovery, manage records, and participate in community meetings.

Qualifications

• Experience providing services to homeless or high-needs clients is required.
• Strong interpersonal and communication skills are essential for outreach and collaboration.
• Ability to conduct assessments, develop safety plans, and coordinate resources.

Responsibilities

• Identify and engage LEAD program clients living on the streets in Pacific Division.
• Collaborate with team to remove barriers to permanent housing and access services.
• Provide case management and aftercare/follow-up for clients in permanent housing.
• Maintain accurate electronic records and attend staff meetings and trainings.
