Job summary

Qualifications

• AA or higher in Drug and Alcohol, Social Work, Human Services or related field.
• Mature Christian faith and alignment with Fresno Mission beliefs are required.

Responsibilities

• Coordinate with member's service providers and support network to ensure seamless experience and avoid duplication of services.
• Actively engage ECM-eligible members.
• Oversee the provision of ECM services and implementation of the care plan.
• Connect ECM members to social services and transportation as needed.
• Advocate for ECM members with health care professionals.
• Monitor adherence to treatments and health promotion activities.
