Job summary

The California Health Collaborative is seeking an ECM-LCM I to evaluate clients and deliver ECM services under the leadership team. You may also handle CalAIM Community Supports caseloads and assist MSSP programs as assigned.

Responsibilities include conducting assessments, developing care plans, enrolling members, coordinating with an interdisciplinary team, and documenting encounters in CalAIM portals. Occasional evenings/weekends may be required.

Qualifications

• Bachelor’s degree or higher in a relevant field and one year of care-management experience.
• Community Health Worker certification is preferred but not required.
• Strong communication, interpersonal, and organizational skills.

Responsibilities

• Evaluate clients and participate in developing care plans and follow-up.
• Deliver CalAIM ECM/CS services and assist with MSSP programs as needed.
• Conduct outreach, enrollments, and documentation in CalAIM portals.
• Collaborate with interdisciplinary teams and ensure timely documentation.
