Job summary

Elevance Health seeks a LTSS Service Coordinator to manage a defined caseload, conducting assessments and supporting Person Centered Planning for members with LTSS/IDD, BH or PH needs.

You will collaborate with care teams, document goals, coordinate services, and serve as the single point of contact for members, families, and providers while traveling to client sites as needed in Johnson County and surrounding areas.

Qualifications

• BA/BS degree.
• Minimum of 2 years of experience with a social work agency.
• State law and contract requirements may dictate education and experience.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS/IDD, BH or PH needs.
• Serve as the single point of contact for clinical care management and member involvement.
• Document short and long-term service goals with the member's care team and support plan.
• Submit utilization/authorization requests and report critical incidents to appropriate parties.
