Job summary

Elevance Health is seeking a LTSS Service Coordinator to manage a defined caseload across Henry, Randolph and Wayne counties. The role combines in-field assessments with care coordination, planning, and documentation in collaboration with the member and care team.

Travel to client sites is expected; occasional office attendance may occur. Requires a BA/BS and at least 2 years in social work or related field, with experience in waiver programs and care coordination.

Qualifications

• BA/BS degree. Minimum of 2 years of experience working with a social work agency.
• Experience with waiver programs (LTSS/IDD), assessments, and care planning.
• Travel to worksites and other locations as necessary, with hybrid/field-based expectations.

Responsibilities

• Conduct face-to-face program assessments using predefined questions.
• Coordinate and manage members' LTSS, BH/PH needs with the clinical team.
• Document goals in the Person Centered Support Plan (PCSP) with the member and care team.
• Submit utilization/authorization requests and report critical incidents to appropriate parties.
