Job summary

Elevance Health is seeking a LTSS Service Coordinator to support clients in Henry, Randolph, and Wayne counties. This field-based role requires travel to client sites, collaboration with care teams, and development of Person Centered Plans.

The position emphasizes coordinating long-term supports, managing health and social needs, and documenting goals with the member and their support network, while ensuring compliance with state and contractual requirements.

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience working with a social work agency or equivalent background.
• Education and experience may be required by state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments and document member goals using person-centered planning.
• Coordinate and manage a member's LTSS/IDD, BH or PH needs with the care team.
• Submit utilization/authorization requests and support in appeals, audits and grievances.
• Identify members who would benefit from alternative levels of service or other waiver programs.
