Job summary

Elevance Health is seeking an LTSS Service Coordinator to manage a defined caseload in a field-based role. Travel to client sites and collaboration with the member, family, and care team are essential to document preferences, needs and goals in the Person Centered Planning process.

You will coordinate assessments, develop the PCSP, and back-up plan, interface with Medical Directors, and participate in interdisciplinary rounds to ensure an integrated care plan.

Qualifications

• BA/BS degree required or equivalent based on state law and contract.
• Minimum of 2 years of experience working with a social work agency.
• Experience coordinating care with interdisciplinary teams as per contract requirements.

Responsibilities

• Conduct face to face program assessments for waiver eligibility and needs.
• Coordinate cases with clinical teams to provide care coordination support.
• Manage non-clinical needs to optimize LTSS utilization.
• Document member goals in collaboration with the care team.
• Identify members who may need alternative services or waivers.
• Support training of new staff and participate in process improvements.
• Submit utilization/authorization requests with care plan alignment.
• Report critical incidents to state/county agencies as required.
• Assist with appeals, grievances, and state audits.
