Qualifications

• BA/BS degree.
• Minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments for LTSS/IDD, BH or PH needs using predefined tools.
• Coordinate member cases as the single point of contact with the clinical team to support care planning.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective LTSS utilization.
• Document member goals in collaboration with the member's care team and circle of support.
• Identify members who would benefit from alternative levels of service or other waiver programs.
• Mentor or precept for new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, state audits, and related processes.
