Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to support Wisconsin IRIS participants remotely, with planned in-person home visits. You will help participants identify goals across employment, housing, health, safety, and community involvement, while navigating IRIS budgets and resources.

The role emphasizes relationship-building, independent work, and collaboration with the IRIS team. You will travel as needed and support diverse populations within the community.

Qualifications

• At least 2 years experience in health care, preferably in care coordination, and at least 1 year serving target IRIS groups (adults with disabilities or older adults).
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, special education, or four years of commensurate experience if no degree.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job travel requirements.
• Ability to develop positive and effective work relationships with coworkers, clients, participants, providers, regulatory agencies and vendors.
• Ability to work independently with minimal supervision and demonstrate self-motivation.
• Demonstrated knowledge of long-term care programs.
• Familiarity with self-determination.
• Problem-solving and critical-thinking skills.
• Excellent time-management and prioritization skills.
• Microsoft Office suite/applicable software proficiency.

Responsibilities

• Meet in person with the IRIS participant a minimum of four times per year, with one required annual visit in the home.
• Provide program orientation to new participants and explain rights and responsibilities in IRIS.
• Develop customized IRIS plans to achieve participant goals.
• Document orientation and planning activities within the IRIS data system (WISITs) within 48 business hours.
• Research community resources and natural supports to fit individual outcomes.
• Document progress/changes in the plan/data system as modifications are requested.
• Educate participants on reading monthly budget reports and assist with provider billing and related tasks.
• Maintain confidentiality and HIPPA compliance.
• Work with IRIS Consultant Agency staff to implement plans and attend monthly team meetings.
