Job summary

Community Health Action of Staten Island, Inc. seeks an Intensive Case Manager to support survivors of domestic violence. This role involves conducting assessments, navigating complex service plans, and providing advocacy and follow-up support.

Qualifications

  • Experience working with culturally diverse and vulnerable populations who have experienced trauma.
  • Knowledge of HIV, HCV, STI, PEP/PrEP and Harm Reduction methods is required.
  • Ability to work independently and collaboratively in diverse teams.

Responsibilities

  • Conduct initial comprehensive assessments and safety planning.
  • Navigate clients through their service plans and provide support.
  • Collect and submit data documenting all case management activities.

Skills

MS Office proficiency

Education

Bachelor’s degree in health, human services, education, social work, or mental health
Associate’s degree in a related field with minimum 3-4 years of experience
