Job summary

Community Health Action of Staten Island (CHASI) is seeking a Case Manager to provide ongoing, documented case management to adults in our harm reduction and recovery program. You will assess needs, develop service plans, coordinate referrals, and track progress to help participants reach their goals.

The role emphasizes trauma-informed, culturally responsive care, collaboration with partners, and accurate documentation.

Qualifications

• High school diploma or equivalent with at least 1 year of case management experience.
• Experience supporting people affected by substance use, housing instability, or related needs.
• Knowledge of harm reduction, trauma-informed care, and motivational interviewing.
• Ability to assess needs, develop service plans, coordinate referrals, and document outcomes accurately.
• Ability to navigate MS Office, email, internet tools, and EHR systems.
• Strong organizational skills and ability to manage multiple priorities.

Responsibilities

• Engage eligible participants through respectful outreach and relationship-building.
• Maintain caseload and provide ongoing case management from enrollment to closure.
• Complete assessments, develop service plans, and monitor progress.
• Coordinate referrals and navigation for medical, behavioral health, housing, benefits, and more.
• Provide harm reduction education and health promotion activities.
• Maintain timely documentation in agency and funder systems.
• Participate in case conferences, supervision, training, and quality assurance.
