Job summary

CWS Durham in Tampa, FL seeks a dedicated Case Manager to provide direct services and trauma-informed, culturally responsive support to clients facing complex challenges. You will conduct assessments, coordinate referrals, document thoroughly, and advocate within community systems to secure necessary resources.

This role emphasizes housing coordination, collaboration with volunteers, and adherence to program protocols while maintaining confidential records and reporting outcomes for grant

Qualifications

• Bachelor's degree in a relevant field required or equivalent experience.
• 3–4 years of case management experience in nonprofit/humanitarian settings preferred.
• Strong understanding of trauma-informed, culturally responsive care.
• Excellent communication, documentation, and interpersonal skills.

Responsibilities

• Conduct intake assessments to determine client needs, risks, and eligibility for services.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management, monitoring progress, and adjusting plans as needed.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Advocate for clients with external agencies to secure necessary resources.
