Qualifications

  • Needs assessments cover physical health (including HIV/STI), mental health, substance use, dental health, tobacco use, education and employment.
  • Assess and plan for domestic violence and trauma as part of holistic care.
  • Use recovery-oriented and trauma-informed approaches to engage clients and develop service plans.

Responsibilities

  • Maintains a full-time caseload of up to 20 clients including family members or support individuals.
  • Prepares intake, service plans and assessments; initiates needed services and sets mutually agreed goals.
  • Develops service plans every six months with clear goals and progress markers; uses motivational interviewing.
  • Ensures all client documentation is accurate and submitted on time.
  • Monitors progress toward goals and supports clients in attending medical appointments and managing medications.
  • Provides crisis intervention and coordinates responses to emergencies or hospitalizations.
  • Establishes linkages with community providers to ensure service access.
  • Collaborates with Iris House departments to ensure integrated, non-duplicative services.
  • Schedules weekly home visits to assess client and family needs.
  • Participates in weekly team meetings and supervision; promotes diversity and anti-discrimination in the workplace.
  • Keeps current on case management practices through ongoing learning.
